Our Services

Preadmission Consultation

$250

  • This one time consult would take place in the patients home or agreed upon meeting place prior to admission to assisted living or memory care. This would be an individualized visit assessing the patient’s needs, explaining the process, and reviewing and completing facility paperwork; including updated prescriptions and provider orders to ensure a smooth transition.

Home-to-Facility Transition Consulting

$500-1200 (depending on complexity)

This package includes a face-to-face preadmission consultation and 30 days of ongoing support after your loved one moves into a facility. Our providers will visit the facility in person after the move to ensure a smooth transition. While this service does not replace your family doctor, it offers immediate management of new behaviors or acute issues that often arise when a patient adjusts to a new environment. Common challenges include new insomnia, worsening behavioral concerns such as anxiety or aggression, and an increased risk of falls, which frequently occur during the initial move-in period. Addressing these issues quickly is essential to prevent unnecessary trips to the emergency room and ensure your loved one’s safety.
